This Cancellation of Event Registration Form must be faxed or e-mailed to the Organiser. Phone cancellations will not be accepted. Full refund
(minus $100 + GST) cancellation fee) will apply for cancellations received on this correctly completed Cancellation of Event Registration Form by
the Organiser no later than 5pm Friday 9 April 2010. No refunds will be made for cancellations received after 5pm Friday 9 April 2010.
If you are unable to attend the Event you have registered for and wish to substitute another Attendee in your place, you must send a correctly
completed Attendee Substitution Form to the Organiser no later than 5pm Wednesday 19 May 2010. A $75 plus GST administration charge will
