used or disclosed. Protected health information is the individually identifiable personal health information
found in your medical and billing records. This information is created or received by a health care
provider, insurance company, or employer, and relates to your past, present, or future physical or mental
health conditions. This information can be transmitted or maintained in any form by The Methodist

duties and privacy practices of The Methodist Hospital and its Medical Staff members with respect to
health information created for services generated at The Methodist Hospital. If you receive services by
your physician or a health care provider at a different location, there may be different health information

The Methodist Hospital and its Medical Staff members are independently responsible for complying with
this Notice. While we may share protected health information with each other to carry out treatment,
payment, or health care operations, the Medical Staff members treat patients at The Methodist Hospital
but are not employees of The Methodist Hospital, and we are not responsible for each other's actions,
and do not have equal control over the other's business.
