Corneal Refractive Therapy Agreement
Congratulations on deciding to undergo Corneal Refractive Therapy (CRT)! You are beginning an
exciting process designed to reduce and possibly eliminate your dependency on eyeglasses or daytime
contact lens wear. Paragon CRT™ therapeutic lenses gently and non-surgically reshape the front
surface of your eye while you sleep.
Carmel Mountain Vision Care is an FDA clinical study site for CRT. Our practice was one of the first in
the nation to offer this revolutionary treatment, beginning in July of 2000. As a result, we have the largest
number of successfully treated patients in CRT in Southern California. Our leadership and experience
with CRT combined with our state-of-the-art corneal mapping instrument, the Humphrey Atlas corneal
topographer, offer you unparalleled CRT care.
DURATION OF TREATMENT
The length of your treatment depends primarily on your prescription and your compliance with the
prescribed CRT wearing schedule. Typically, we expect to see about 90% of the vision changes within
the first week of treatment and full treatment within two weeks. Some eyes will require a longer period of
time to complete the process. Thereafter, you should wear the therapeutic lenses routinely to maintain
Our global fee for CRT is $1,950.00 for both eyes and $1,175.00 for treatment of one eye only. This
global fee must be paid in full before your Paragon CRT therapeutic lenses are dispensed. The fee
includes all professional services and any changes in the Paragon CRT therapeutic lenses to complete
treatment for a period not to exceed six (6) months from the date of initial lens dispensing. This fee also
includes lens solution for the prescribing period and a “starter kit” with a suction remover, a lens case,
