An Applicant’s Guide to Completing the
Amateur Swimming Association Member
This booklet contains step-by-step notes on
how to fill in the registration forms for category 2 (blue)
and category 1 and 3 (pink).

•Use black ink throughout and write clearly in BLOCK CAPITALS only.
•Use only one letter or number for each box.
•Leave an empty box between words.
•Infill choice circles where indicated (please see example)
•Be sure to sign the form where indicated.
•Make sure you retain the back copy of all forms for your records.
•Don’t write over the edges of the box.
•Don’t place any stickers or staple any other paper to these forms.
•Don’t use correction fluid.
•Don’t fold the forms.
Your step by step guide to filling in category 1 & 3 form
Most of the information on the pink form is already explained
on the guidelines for the category 2 form, please refer to this
for detailed explanation on filling in the names, addresses and
the ethnicity parts of the form. The differences are shown
and explained below.

Please note the list of clubs of which you are a member
must be completed in chronological order, i.e. in order of
the longest continuous membership. Again the club code

Member registration form. Items marked with an *
are mandatory and therefore must be filled in. If you
do not complete the mandatory fields your form may
be returned to you. Items not marked will assist the
Membership services team to process your form.
