ODRC FINDINGS ON THE DELAYED 911 CALL
AT THE LONDON CORRECTIONAL INSTITUTION
ON FEBRUARY 4, 2004
Special Incident Report
On February 4, 2004, the CIIC staff office received the ODRC Special Incident Report on
the death of Kareem Benjamin 325-212. The time of the incident was reported to be 6:00
PM. The incident was described as follows:
Inmate Benjamin 325212 was at Recreation working out and
collapsed. Medical staff arrived at Recreation and started working
on him. Inmate Benjamin was moved to I.H.S. where the medical
staff continued to work on him until the squad arrived. Inmate
Benjamin was transported to Madison County Hospital where he
was pronounced dead at 7:35 PM .
The Special Incident Report cites the Nature of the Incident as, “Inmate Death, Natural
Causes.” It also reports that the Highway Patrol, Coroner, and next of kin were notified.
Further, the report states that Kareem Benjamin entered the prison system on December
19, 1995, with a parole eligibility date of April 21, 2007. His current age was reported to
be 28. According to the Incident report, autopsy arrangements had been made. In
follow-up communication from the LOCI Administrative Assistant on March 24, 2004, it
was reported that the Death Certificate cites pulmonary embolus and cardiac
hypertension as the cause of death.
ODRC Security Administrator’s Report
Per a CIIC member’s request at the CIIC meeting on March 17, 2004, an inquiry was
made to the Deputy Director of Institutions on March 22, 2004 to seek information on the
DRC findings in the investigation into the reported delay in calling for the medical squad
for Kareem Benjamin on February 4, 2004. The ODRC report was faxed to the CIIC staff
office on March 22, 2004. Per the report, on February 9, 2004, the Central Office
Regional Security Administrator was assigned the task of determining why there was a
delay in getting an ambulance into the London Correctional Institution on February 4,
2004 when Inmate
