This letter is written with the permission of the patient Daniel Kauwe DOB 5,12.1977.
Me Kauwe is under disability consideration due to his HIV. I don't have information on
the initial claims.
He has HIV and will need medical treatment soon. This will be likely a once a day set of
medications which are usually well tolerated though can have some minor side effects.
We have discussed the treatment options and plan to initiate this soon.

The patient states that he can do administrative work but not physical labor for a
prolonged period of time. He is aerobicallywell as he biked 70 miles from Woodland
Hills to Costa Mesa CA this week. From a medical standpoint, there is no limitations to
his work activities under the information that I have available.
With his upcoming treatment, I recommend that there be accommodations for time off if
there are any unexpected side effects as well as for our regular appointments in Century
Cityin Los Angeles.
Currently he is not on any medications.
I have referred him to cognitive behavioural therapy with James Peck in Santa Monica
CA as well as a psychiatrist Ali Aziz in Beverly Hills CA.
He mavreturn to work.
