Automatic Investments and
Electronic Funds Transfer for IRAs
Please use this form if you would like to set up Electronic Funds Transfer (EFT) and/or Automatic Investments to your Fidelity
IRA, including Traditional or Roth IRAs. If you would like to set up contributions for more than one IRA, please photocopy
this form and provide an original signature on each form. This form is not intended for setting up exchanges within an IRA,
as exchange restrictions may apply.
Electronic Funds Transfer (EFT) allows you to transfer money electronically from your bank to your Fidelity IRA. The minimum
additional EFT contribution amount to a Fidelity mutual fund is $250 or the fund’s minimum, whichever is greater. See prospectus
for details. Please allow three business days from the date you request an EFT contribution for the funds to transfer. To add EFT
only, complete sections 1, 2A, and 4 only.
Automatic Investments lets you choose a monthly, quarterly, or semiannual date for automatic purchases to your eligible Fidelity
account(s). Transfers must be for a minimum of $100 (some funds may have higher minimums). For contributions into a new fund
in your portfolio, you must establish Automatic Investments for $200/month. Refer to the fund’s prospectus for Automatic Investments
NOTE: Fidelity does not charge a fee for automatic investments in Fidelity funds or No Transaction Fee (NTF) FundsNetwork® funds.
After the initial investment, a $5 fee is charged per automatic investment into a FundsNetwork transaction fee fund. Other regular mutual
fund fees may also apply. To add Automatic Investments to your IRA, complete all sections of the form.
Allow at least 10 business days for these services to be activated after we receive this form. To change the amount, frequency, or date of
a contribution, or to skip a contribution, call Fidelity at 1-800-544-6666 or visit www.fidelity.com/autoinvest at least five business days
before the scheduled investment date.
