We've created a valuable resource to spot potential
medication errors and misunderstandings with our revised
"Brown Bag Review" services.
Our goal is to ensure that everyone in our
community from the single parent to the family on a
fixed income to aged seniors with memory care
issues, stay on top of their health and safety and
avoid risk! However, it remains a fact for all of us
that medication reconciliation is often challenging
So we all need to be aware that medical record reviews can't
guarantee that our senior loved ones are adhering to their
medication regimens.
Studies have shown that medications prescribed at
home can be missed or stopped without follow-up.
Remote patient assessments are more prone to miss
opportunities for dosage reductions. And unreviewed,
medicines and supplements taken together may
conflict with each other causing patient harm.
So these problems are addressed head-on by our new, improved
prescriptions and supplements review service ensuring that all
involved have time to think through issues raised and decisions
made.
The need for our Brown Bag Reviews is not going
to go away. According to the Alzheimer’s
Association, 1 in 7 seniors aged 71 and above
have a form of dementia or memory care issues.
They need to be on top of their prescriptions since
they are more susceptible to chronic illnesses.
The situation is predicted to worsen as the US population of 65+ is projected to reach 80 million in
2040, with four times as many over 85 needing help. The number of 35-year-olds with Alzheimer's
