To sign up for direct deposit of your commissions:
1. Complete the following information
2. Include a voided check from the account in which commissions are to be deposited.

•  To get daily deposits, you must have sold a membership within the calendar month.  Daily deposit begins aft er your 
 personal sale for the month is processed, and commissions must exceed $25.00 for the day to be deposited. 
•  To get weekly deposits, you must have sold a membership within the calendar month.  Weekly deposits begin aft er your 
 personal sale for the month is processed, and commissions must exceed $75.00 for the week to be deposited.  
•  Monthly deposits process when commissions exceed $150.00 for the month to be deposited.
