Telephone: 404 - 347-9996 Fax: 404-347-9878 DIVORCE DOCUMENT LIST Note: Any reference to "you" includes situations in which you share ownership or are jointly liable on a debt or obligation with any other person, including acting as a trustee or custodian. If you are in possession of any of these items relating to an asset belonging to the other party, please bring it to mediation. 1. All pay stubs, statements of earnings, Forms W-2, 1099, or K-1 given to you showing income from any source for the past three whole years and the current year-to-date. 2. Your federal income tax returns for the past five years including all attachments, and any tax estimates you may have filed for the current calendar year. 3. Cash account information including the most recent monthly statements and passbooks from all checking, money market or savings accounts maintained by you with any bank, brokerage house, credit union or other financial institution. 4. Current statements or information about your debts, showing: (a) The total amount of each and every debt you owe, and any security held for such debt; and (b) The total balance owed and the required monthly payment on each such debt. 5. Any statements or documents you may have showing: (a) The current balance of the first and second mortgages on your home; and (b) The present fair market value of the home. 6. Evidence of ownership of securities, as follows: (a) Certificates for any stock, bond or mutual fund, brokerage account statements, or other evidence of ownership of any interest in any corporation, fund, trust, or other investment vehicle; and (b) The latest monthly, quarterly, annual or other statements from any stockbroker, investment counselor or financial institution relating to your or your spouse's interest in any security. 7. The most recent statement of the account balance and/or accrued benefit in each qualified pension, profit sharing, 401(k), stock bonus plan or Individual Retirement Account in which you or your spouse has any interest. 8. Records of motor vehicles, aircraft and boats owned by you or your spouse: (a) Title certificates and tag registration receipts (b) Motor vehicle leases or lease/purchase agreements; (c) Evidence of policies providing insurance coverage for any of said vehicles. 9. Policies, plans or employee benefit booklets describing any health insurance now covering you, and any of your children. 10. Life insurance policies and annuity certificates, naming you as an owner, an insured or a beneficiary, and the most recent annual statement issued by the carrier on each such policy. 11. Any personal financial statement or net worth statement prepared or submitted by you over the past five (5) years. 12. Records of any interest in real estate you own, including specifically: (a) Warranty deeds, quit-claim deeds and deeds to secure debt which name you (individually or with others) as grantee or grantor; (b) Closing statements, contracts of sale or other documents setting forth the terms of any purchase or sale; (c) Leases or lease/purchase agreements showing you as either a lessor or lessee; and (d) Real estate option agreements to which you are a party. 13. Financial records of any closely held business in which you own an interest, including specifically its: (a) Last three years= federal tax returns; (2) Financial statements for the past three complete years and the current year to date, including (i) Statements of profit and loss (income statements) (ii) Balance sheets 14. The contents of any safe deposit box maintained by you, individually or together with any other person. 14.
