Completing the Specialist Acceptance Process
This process applies only to PDS applicants that have successfully completed training and passed the
final examination. If the PDS applicant has not completed the training and exam, s/he does not have
access to the Specialist Acceptance process.
Process Overview
PDS selects the PD Specialist Application tab:
Next: PDS applicant clicks “Details”.
Then, click “Sign PDS Acceptance Documents”
Then, click on “Click here to access and complete your W9” which takes the user directly to the form.
This form must be completed online by the PDS applicant. The PDS applicant must use the same name
included on their application. Once completed, YourCouncil uploads the form. There is no need to print
this form.
The next step is to complete the PDS Agreement (only for independent contractors or volunteers). This
is completed by selecting “Click here to access and sign PDS Agreement”.

Once the PDS Agreement has been selected, the user must select “Get Started” to begin completing
each of the required sections. This is a six (6) page document. The last page requires an electronic
signature. The PDS applicant must use the same name included on their application.

Once electronically signed, the signature page looks similar to this:
Once completed select “Done” and the Agreement is uploaded by YourCouncil.
YourCouncil will endorse only after the user selects “Summary Status” from the menu on the left and
then clicks “Confirm Your Endorsement” once on the Summary page.
Once confirmed the application status changes to “Endorsed”
